Tissue Doppler Strain (SRI) has been previously validatedin the quantitative assessment of ischemic heart disease.2D strain is a novel technique that allows rapid offlineanalysis with automated tissue tracking capabilities andreduced angle dependency. We sought to examine therole of 2D strain in diagnosis of coronary artery disease.Eighty-five patients (age 63 11, 13 female) with known orsuspected coronary diseaseunderwent Dobutaminestressechocardiography and coronary angiography. 2D strainrate (SR) and end-systolic strain (ESS) were measured atrest and during peak stress in those segments supplied bythe left anterior descending coronary artery (LAD).Atotalof (935) segments supplied by the LAD territory were analyzed.The patients were divided into 2 groups accordingto severity of LAD stenosis. Of 85 patients, 31 had significant LAD stenosis >75%. Those LAD territory segmentswith significant LAD stenosis had significantly lower resting SR, peak SR, resting ESS, and peak ESS (Table). Thearea of ROC curve was 0.91 for peak SR and 0.88 for peak ESS to diagnose LA Dstenosis of≥75%. Therewas good intraobserver(r = 0.91, p < 0.001; mean difference 0.00 0.27)and interobserver variation (r = 0.82, p < 0.001; mean difference 0.03 0.40) for 2D strain measurement. Therefore,2D strain is a new technique that is useful in the detectionand quantification of coronary artery disease. Measurementswith 2D strain are both accurate and reproducible.
